Deductibles, copays, and co-insurance must be paid at the time of the examination. If you have any questions regarding insurance coverage, eligibility, or an exam authorization, please contact us for assistance.
Questions and Answers
We keep expanding our network of health insurance plans. When applicable, we will accept your benefits. However, you are still eligible to for our Self-Pay Patient Program
When you request an appointment, we will need your doctor`s order, insurance card, and a form of ID.
We contact your insurance plan to verify your eligibility. Depending on your plan, deductibles, copays, and co-insurance must be paid at the time of the examination.
The eligibility verification may take 2 business days
Co-pay, deductibles, and coinsurance must be paid at the time of the examination. We will collect amounts equal to your in-network benefits and will notify your insurance plan accordingly.
Check with your insurance plan to determine if an authorization is necessary as you could be responsible for all charges if a proper authorization or a referral is not obtained.
In addition to insurance eligibility verification, for all MRI and CT exams you will need an authorization number approved by your doctor and insurance.
If you need our help to obtain an MRI or CT authorization for you, the authorization process may take 5 business days, provided that your doctor’s office and insurance cooperate.
